ABSTRACT: Myopericytoma is a rare mesenchymal neoplasm arising from perivascular myoid cells, typically affecting the limbs of middle-aged adults and only occasionally involving the oral cavity. We present the case of a 49-year-old man with a painless, slow-growing pedunculated mass on the tongue, which on excision proved histologically and immunohistochemically consistent with myopericytoma. The lesion was completely excised with no evidence of recurrence, supporting the generally benign course of this tumor. Given its rarity and nonspecific presentation, this case underscores the value of histopathological and immunohistochemical evaluation in reaching an accurate diagnosis of intraoral spindle cell lesions.
