  ABSTRACT: Tizanidine withdrawal syndrome is an infrequently reported condition caused by the abrupt cessation of tizanidine, a short-acting central α2-adrenergic receptor agonist. This case details a 38-year-old female who presented with moderate withdrawal syndrome induced by the abrupt discontinuation of high-dose tizanidine. The patient presented with autonomic and adrenergic hyperactivity, neuromuscular rebound, and neuropsychiatric disturbances. Management utilized multiple medications targeting hyperadrenergic and autonomic hyperactivity. This was followed by the reintroduction of tizanidine at a lowered baseline dose, which was then gradually reduced using a structured down-taper. The case illustrates an ambulatory detoxification protocol useful for tizanidine withdrawal management.
