ABSTRACT: Coronary bifurcation lesions are anatomically complex and remain challenging in percutaneous coronary intervention (PCI), particularly in side-branch (SB) management and the prevention of in-stent restenosis (ISR). Drug-coated balloons (DCBs) based on the concept of “leave nothing behind” allow localized delivery of antiproliferative agents and inhibit neointimal hyperplasia, which offer novel therapeutic strategies for bifurcation lesions. This review summarizes the principles of DCB technology and the current evidence supporting their application in coronary bifurcation lesions, to inform optimal intervention procedures.
