ABSTRACT: We report a 63-year-old man who developed massive splenic infarction two months following COVID-19 infection. He presented with high grade fever, weakness, dysuria and chronic productive cough. He was found to have a large splenic infarction involving most of the spleen. We document the course and outcome of his illness, and the histopathological findings of the resected spleen. A discussion of COVID-19 associated hypercoagulability is presented, with a comprehensive review of the literature on splenic infarction associated with COVID-19 infection.
