  ABSTRACT: We are describing a rare occurrence of a perforated diverticulum of the fourth portion of duodenum and surgical repair. Patient expired despite prompt surgical intervention secondary to septic shock, multi-organ-system failure (MOSF) and delayed presentation of the disease.
