Chronic Disease Prevention and Management encompasses research and practices aimed at reducing the incidence, progression, and impact of long-term health conditions through prevention, early diagnosis, effective treatment, and continuous care. It covers topics such as cardiovascular diseases, diabetes, cancer, chronic respiratory diseases, obesity, nutrition, physical activity, lifestyle modification, mental health, health promotion, disease surveillance, patient-centered care, healthcare delivery, and public health policy. The field integrates clinical research, epidemiology, behavioral science, public health, and health informatics to improve health outcomes, enhance quality of life, support healthy aging, and strengthen the effectiveness of healthcare systems.
